On April 8, 2026, Phan Chau Trinh University of Medicine Hospital held a training session titled “Proper Awareness of Medical Incidents – Building a Culture of Incident Reporting”, with the participation of the hospital’s healthcare staff.

The program was conducted by:
- MSc. Phan Thi Thao Nguyen – Lecturer, Faculty of Hospital Administration, Phan Chau Trinh University; Quality Management Department, Phan Chau Trinh University of Medicine Hospital
- Mr. Ho Hoang Nhi – Quality Management Specialist, Phan Chau Trinh University of Medicine Hospital
During the session, participants were introduced to key contents:
- Proper understanding of medical incidents from a systems perspective, not limited to individual errors
- The role of incident management and reporting culture in the healthcare environment
- Identification of common incidents in clinical practice
- The hospital’s medical incident reporting process
More than just a professional training session, the program also contributed to fostering a proactive – transparent – non-blaming incident reporting culture, thereby enhancing patient safety and quality of care.
This activity is part of the roadmap to strengthen and improve the quality management system in accordance with AACI standards, aiming toward increasingly higher benchmarks in patient safety.
