The Pragmatic Clinical Trial: Reconciling Intention, Design, and State of Mind

What makes a clinical trial a "pragmatic" trial? Since the concept of pragmatic trial research was introduced nearly 60 years ago, this label has been applied and interpreted beyond its original meaning.
